I don't know what's a "cmn " for diabetic supplies. However, diabetic supplies are available at most, if not all, pharmacies. If you are referring to insulin, it depends on the state law where you live. Some states require a prescription to get insulin. Most states require a prescription to get syringes to inject insulin. If you're talking about a certificate of medical necessity, then the medical supply company should be able to provide the form and help fill it out. Then your doctor would need to sign it.
